Dear Parent/Guardian,
To ensure the health, safety, and well-being of your student, it is essential that the Request for Special Dietary Accommodations form be completed and submitted if your student requires any special dietary restrictions. This form serves as the primary means of communication between the School Nurse, the Food Services Department, and your family regarding specific allergies or medically necessary food substitutions.
Please be advised that, in accordance with district policy and federal regulations, the district is required to have this form on file—fully completed and signed by a recognized medical authority—before any modifications or substitutions can be made to your student’s meals. This procedure is in place to safeguard your student’s health while ensuring compliance with federally mandated nutritional standards.
Your student’s safety remains our highest priority. In the absence of a completed and approved form, your student will receive the standard meal options available to all students during breakfast and lunch service. Due to the strict federal nutrient requirements that govern school meal programs, Nutrition Services is unable to accommodate food substitutions based on individual preferences, including those related to religious or philosophical beliefs. We encourage parents and guardians to assume responsibility for managing and monitoring dietary restrictions connected to these practices outside of the meal program.
We kindly request that you have your healthcare provider complete the Request for Special Dietary Accommodations form and return it to the school at your earliest convenience. Upon receipt, the district will be available to meet with you to discuss how we can best support your student’s nutritional needs and ensure they receive meals that are both safe and nutritionally equivalent to the standard offerings.
